Carers Respite Offer

We are pleased to introduce our new STAR project - Supporting Time Away and Respite (October 1st - March 31st 2025).

We are excited to announce a new project aimed at providing respite for unpaid carers at short notice. Respite options will include residential placements for their cared-for individuals (lasting up to one week) and shorter, bespoke respite opportunities through home care and sitting services. This project is designed to support carers by allowing them essential time for rest and rejuvenation at short notice.

To access this provision you must not be accessing any other respite provision from Adult Social Care.

Eligibility and Referral Process: Carers who meet the criteria will be identified through the CCS Advisor Team, Admiral Nurse Service, and applicable ASC teams, as well as through self-referral. ASC team members will facilitate referrals for a holistic assessment, ensuring a seamless connection to the respite offer.

Personalized Support: Each carer will collaborate with our team to create a personalized support plan, identifying their preferred options . This tailored approach ensures that carers receive the specific support they need.

Ongoing Support and Feedback: Carers will be connected to the necessary support services, with a clear process established for providing feedback. Additionally, the wider service offer will be made available and implemented as required. Referrals for carers’ needs assessments will be actioned promptly, and updates will be recorded on the carers record. Follow-up contact will be made to gather feedback and ensure ongoing support.
